Friends, I am not a registered retailer under GST. I have a retail store for Gifts and Chocolates.
1. What should be my Invoice Format ?
2. Do I need to mention prices are inclusive of GST ?
3. Any additional information that I have missed
Please guide me and if possible provide me with a sample JPG file link so that I may proceed further.

You can design you own invoice. As you are not registered under GST, no format applies to you.
You can randomly search Google for invoice format and use that.
But keep invoice numbers serial wise to prove that your sales are not more than 20 lakhs in a year.
